Person Centred Planning (other professionals)- Session 4

Welcome to our online training session on Person Centred Planning for professionals working with individuals with Special Educational Needs and Disabilities (SEND).

This course is made up of FOUR sessions, all of which should be attended to complete the course.

  • Session 1 : Friday 20th September 9.30 – 12.00
  • Session 2: Friday 27th September 9.30 – 12.00
  • Session 3: Friday 4th October 9.30 – 12.00
  • Session 4: Friday 11th October 9.30 – 12.00

Workshops are held via Zoom and invites and log on instructions will be emailed to you prior to the course starting.

Course Outline

Excellent person-centred education, health and care planning results in clear person-centred outcomes for children and young people receiving support. Outcomes that are developed from an understanding of what matters, who matters and where matters to them and what is important to them in the future.

This programme is designed to support participants in developing clear, specific outcomes that are

  • rooted in quality person centred information generated through person-centred thinking and person-centred conversation. The programme supports participants to explore creative ways to help the child or young person achieve their outcome in collaboration with the people who love, care for, and support the person, helping them move towards a future they want by exploring the person’s own strengths and qualities and what is available to them including technology, community, and services.
  • The programme enables everyone involved in supporting the child or young person to develop well-formed measurable person-centred outcomes using The Outcome Sequence and explore with creativity the ways that these can be achieved by using The Support Sequence.’
  • ‘This training is for professionals involved in working with and supporting children/young people in Person-centred Reviews and using The Outcome Sequence and The Support Sequence.
  • This programme is designed to support professionals to better understand what matters to each person they support, how to develop clear, specific outcomes together and look at creative ways to achieve outcomes that includes technology, family, friends and neighbours, community, and services.’
