Complex health team

We are a small team of experienced nurses with backgrounds in children’s nursing, children’s mental health, health visiting and children’s long-term conditions.

We work typically between the hours of 8am to 4pm Monday to Friday.

What we do

We provide a range of support for children and young people who are identified by assessment as having high-level complex health needs.

This may include health support through continuing health care, including a personal health budget. For children and young people aged 0 to 18 years.

Who is in the team

Designated clinical officer – The Designated clinical officer works as part of the team. This is a varied role and plays an important part in supporting children and young people 0 to 25 years with SEND to have the correct health support to achieve their goals and aspirations.

Children’s nurse educator – Our children’s nurse educator supports children, young people, and their families in ensuring carers or teaching staff are confident, competent and understand the management of the young person’s clinical needs. This allows them to access settings and recreational activities of their choosing. For children and young people aged 0-18 years

Complex health nurse – The complex health nurse reviews and update packages of continuing health care, to ensure they remain effective and are working for the child and their family. They also provide advice and support for children and young people with complex clinical health issues in mainstream education settings for children and young people up to 18 years of age.

Who we work with

We have a nurse in school who works in Humberston Park special school supporting children’s complex health conditions.

All of our team work closely with education, social care and St Andrews Hospice children’s unit ANDYS in ensuring individual needs are identified and outcomes are achieved. We work closely with adult continuing health care and ensure a smooth transition to adult continuing health care is made where appropriate.

For continuing health care, any health professional you are involved with can make a referral by completing the continuing health assessment with you and your parents or carers. You or they may wish to ring for advice on 01472 323660.

Anyone can ring and ask for advice around complex health conditions or training.

The Designated Clinical Officer is available on 01472 323166. They sit alongside the special education needs and review team to ensure a smooth process is in place.
